Every three years, the Bexar County Health Collaborative partners with local hospitals, community organizations, and residents to conduct a Community Health Needs Assessment (CHNA). The CHNA combines data analysis with community input to identify priority health needs and guide collective action and investment.
The 2025 CHNA marks a regional milestone, with assessments released concurrently for five locations —Bexar, Atascosa, Comal, Guadalupe, and surrounding Texas Hill Country communities.
Each of the 2025 CHNAs was developed through a community-centered, equity-driven, mixed-methods process. Data collection included analysis of existing quantitative data, community resident surveys, local focus groups with priority populations, and interviews with key informants serving communities with the greatest needs. Special emphasis was placed on engaging individuals with lived experience and centering voices most impacted by health inequities.
Each assessment examines a wide range of demographic, social, economic, and health indicators, as well as community strengths. Findings are intended to serve as a shared tool across sectors—including healthcare, public health, education, housing, workforce development, philanthropy, and advocacy—to inform funding decisions, strategic planning, program development, and system-level change.

While each CHNA reflects the unique needs and priorities of its counties, together they provide a shared regional perspective that supports cross-county collaboration, alignment of strategies, and opportunities to strengthen local health.
Recognizing that health does not stop at county lines, the Bexar CHNA is presented alongside the other county assessments. Stakeholders are encouraged to consider findings across all assessments when designing programs, allocating resources, and building partnerships that improve health and well-being throughout the region.
