The Baptist Health Foundation of San Antonio has awarded one year of continuation funding to the Presa Community Center for Project Puente, a promotora/community health worker program to prevent immediate hospital readmission by providing low-income and senior people with heart failure – as well as their families and caregivers – with the knowledge, skills, and tools to successfully manage their condition. CI:Now provides data and evaluation support to Project Puente.
